That way, you’ll be able to create a plan that helps ensure you get the care, treatment and support you need — and deserve.

Ready to get started? Here’s how it works: Download and print the Renew Checkup Challenge Activity Guide. You’ll use this to complete mini self-assessments, quizzes and checklists. There’s also plenty of room to jot down notes and questions. At the end of the challenge, schedule your next checkup (if you haven’t already) and bring this guide with you to share with your PCP.
